Pretty easy to do if that shotgun was in riot configuration or something similar. Guy was prolly pissed and started firing off rounds, placing his hand too near the end of the barrel while firing round number 2. . . I've seen this when people who do not use firearms very often first use a scatter gun in riot configuration. They reach out to steady the weapon on the underside of the barrel and put their meat hooks far too close to the business end of the thing. 

Some cheaper versions of scatter-guns can go off by bumping them too. Especially the older ones.

Take the Remington 870 express I own as an example. An after market kit can be purchased that replaces the original stock, fore-grip, barrel, and round chamber, with a pistol grip, shortened barrel, extended round chamber, modified fore-end grip, and heat displacing sheild, to make it just as pictured in the previous post.

Some arms manufacturers sell shotguns already in this configuration to law enforcement entities and for personal protection purposes as well. These are typically sold in some federally licensed gun shops and by some dealers. The kits are sold by most outdoors outfitters as well as gun shops, dealers, etc.

These kits keep the weapon within legal limitations but allow it to be utilized for riot duty or personal protection purposes.

Not much left of the original after modification.  Looks like for personal protection it would be dangerous for  shooting off one's foot.  How do you conceal it?

Attach a rope loop to the pistol grip and put your arm through the loop, allowing it to hang under the arm under a long jacket or coat.
